Personal Data Consent Statement for Job Applicants

Collections of Personal Data

MEDICENTRO HEALTHCARE SDN BHD ("MEDICENTRO") collects your personal data throughout the course of this recruitment purpose. The personal data which has been or will be collected includes but is not limited to:

i. Your personal details which may include but is not limited to name, gender, date of birth, home and correspondence address, contact details, phone number, identity card number, nationality, immigrant status, visa and passport information, education background, driving licence number, income tax reference number, bank account number and your financial and/or bankruptcy status;

ii. Any sensitive personal data which may include but is not limited to information relating to your religious belief, medical and health related data and information relating to the commission or alleged commission by you of any offence;

iii. Details of any person(s) identifiable from your personal data which may include but is not limited to information about your dependents; and/or

iv. Other information about you, from which you are identifiable,

(collectively referred to as “Personal Data”).

Where you have provided Personal Data of another individual, you are to ensure that you have informed them that you are providing their Personal Data to us, and have obtained their consent to do so.

Purposes

The Personal Data you provide in the job application form, resume, curriculum vitae and any document submitted by you and Personal Data which is provided by third party information sources to MEDICENTRO including your former employer, or referees and secured from back ground checks, in this recruitment process will be collected, used, disclosed, processed and/or transferred by PPRM for the purpose of processing your job application, including but not limited to/for:

1. register you as a candidate for employment;
2. communicate with you;
3. evaluate your suitability for the position;
4. conduct background and reference checks;
5. prepare your offer of employment;
6. submit to the relevant government or regulatory agencies;
7. general and administrative purposes;
8. general administrative, storage and record keeping purposes
9. such other purposes as may be required by law;
10. the exercise of any functions conferred on any person by law and/or towards the administration of justice; and/or
11. any purpose incidental, ancillary or in furtherance to the abovementioned purposes,

(collectively referred to as “Purposes”). Please note that it is mandatory for you to provide the Personal Data marked with asterisks in the relevant section of the job application form. In the event you do not provide such Personal Data, we may not be able to process your application.

If you hereafter do not consent to or wish to limit MEDICENTRO’s processing of your Personal Data or the Personal Data of any person which may be identified from the Personal Data provided to MEDICENTRO for any of the Purposes, please notify MEDICENTRO via the contact details stated below. If you do not consent, excessively limit or if you revoke your consent to us processing your Personal Data, we may be unable to process your application for employment.

Disclosure to Third Parties

MEDICENTRO will keep your Personal Data confidential but if and when necessary, your Personal Data may be disclosed to the third parties including the below for any of the Purposes or as any such purposes as updated from time to time:

i. authorised agents, suppliers, contractors and third party service providers who provide services to MEDICENTRO and/or companies within the group of companies which MEDICENTRO belongs to (“Group Companies”);
ii. external professional advisors, auditors and tax agents;
iii. governmental departments, authorities, any regulatory body;
iv. any business and service partner, investor, third parties conducting background checks, assignee or transferee;
v. new potential employers should there be a corporate restructuring;
vi. liquidators or administrators of MEDICENTRO;
vii. any person authorised by or under any law or by the order of a court; and
viii. any Group Companies,

(collectively referred to as “Authorised Third Parties”)

Retention

MEDICENTRO will not retain your Personal Data for longer than is allowed by law or is necessary for the Purposes.

If you are selected and offered employment, the Personal Data you have provided will be used by MEDICENTRO for the purpose of human resource management and administration. If you are unsuccessful on this occasion to secure employment with PPRM, PPRM will retain your Personal Data for one (1) year for the purpose of considering you for other suitable positions as and when they arise. If you do not wish to be considered for other suitable positions by MEDICENTRO, please indicate accordingly in the acknowledgment section.

Transfer

MEDICENTRO may transfer your Personal Data to Authorised Third Parties which may be within or outside of Malaysia, including to Japan, Singapore, Australia, and other countries in which the Group Companies are located. In the event that your Personal Data is transferred to a location outside of Malaysia, MEDICENTRO shall take reasonable steps to ensure that reasonable confidentiality safeguards have been put in place to ensure your rights to Personal Data protection remains unaffected.

Inquiries, Complaints, Access and Correction

The Personal Data you provide to us must be accurate, complete, not misleading and kept up-to-date. Should you be made aware of any inaccurate, incomplete or misleading Personal Data or where the Personal Data provided to us earlier have become incorrect or out of date, kindly notify us via the contact details provided below.

Except as provided by law, you have the right to request access and correct your Personal Data, to make any inquiries or complaints and to withdraw your consent given to us hereunder. Such request however, may be subject to the requirements in Personal Data Protection Act 2010 and MEDICENTRO’s request procedure.
